* *Territory 2*: Boston, Brighton, Dorchester, Quincy, Roxbury, Cambridge

*Responsibilities:*


* Provide skilled nursing care based on physician orders


* Assess patient conditions, monitor treatments, and update care plans


* Educate and support patients and families on care management


* Coordinate with healthcare teams to ensure comprehensive care


* Supervise home health aides and conduct regular evaluations




*Requirements:*


* Licensed RN in MA with 1+ year of experience


* Comfortable traveling to clients via personal vehicle
