This position will be responsible for providing assessment of patients home health care needs and coordination of referrals to home care and hospice.
Responsibilities: Heres how youll move healthcare forward in this role: Take part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety. Assessand evaluate the needs of patients with multiple disease processes and comorbidities and develop an appropriate plan of care Provides education and strategies for patients and families Coordinatesreferrals to home care to promote continuity of care Partners with other VNA clinicians and external providers to execute best patient outcomes Documentsall changes in patients condition and updates care plans and outcomes Qualifications: The ideal qualifications for the RN Home Care Intake Coordinator include: RN; Bachelors degree in Nursing preferred LPN; Graduate of an NLN accredited LPN program with current NJ LPN licensure Valid NJ nursing license 2+ years of community healthnursing experience Excellent verbal and writtencommunication skills Excellent interpersonal andorganizational skills Strong client focus, assertive,decision making skills, articulate, and creative in problem solving Strong decision making skills Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and drivers license
