RN Assessment Coordinator Responsibilities Conduct and coordinatethe completion of MDS assessmentsMaintain and updateall care plans and assessmentsExamine and reviewresidents' medical historyInform other nursingstaff personnel of any persistent issuesParticipate in the admissionprocess while assessing prospective residents' nursing needs and appropriateplacementCoordinate casemanagement processEnsure that thefacility adheres to all regulatory requirements and standards of practice inrelation to the MDS care planning processCooperate with inter-departmental personnel to ensure services can be adequately maintainedto meet the needs of the residents RN Assessment Coordinator Requirements Valid, current RNlicensePrevious experience inlong-term careOne year job-related experience in the MDS processFamiliarity withvarious computer softwareCOVID-19 Vaccination What We Offer Competitive pay scaleMedical and dentalcoverageReferral bonus401kPaid time offCEU-eligible trainingprogramsOngoing leadership andprofessional development trainingDedicated and localleadership support
