Job Description:

The Infusion Coordinator provides nursing care and coordination for specialty Infusion patients. They provide a personal contact for scheduling, information, and guidance in a compassionate, caring environment throughout a patient's special infusion health care experience. They also perform appropriate coordination with outpatient services. They are to coordinate and track regularly scheduled nursing visits. Track and/or obtain authorizations and reauthorizations for infusions and nursing visits as needed. Provide infusion treatment plans to physicians and maintains communication log. Track outside agency visits by month. Contact outside agencies for provider and/or nursing notes as needed. Collaborate with intake, nursing, and pharmacy staff to schedule visits and orders. They perform appropriate coordination with outpatient services.

IV Infusion certification required.
