Job Title: Speech-Language Pathologist Job Specialty: Rehabilitation - General Rehabilitation Job Duration: 13 weeks Shift: 8-hour Day Shift (40 hours per week) Guaranteed Hours: Not specified Experience: Minimum 1 year of SLP experience (home health experience is ideal) License: Active Illinois license Certifications: Active BLS (American Heart Association) Must-Have: - Master's degree in Speech Pathology- Experience with EPIC is highly preferred Job Description: - Provide quality care to adult patients requiring speech therapy- Participate in patient and family education- Coordinate care and collaborate clinically- Maintain positive patient relationships- 1:1 patient ratios- Receive up to 1 week of orientation based on comfort level- Acceptable attire includes any color scrubs or casual professional wear- Receive support from other case managers, leadership, education team, and team lead manager- Service coverage includes Lake County and parts of McHenry County
