Position Summary:

Are you committed to supporting individuals with serious mental illness (SMI) on their journey to recovery and stability? As an SMI Case Manager, you will play a vital role in coordinating comprehensive care and services for individuals with SMI. You will work closely with clients, their families, and multidisciplinary teams to assess needs, develop and implement individualized service plans (ISPs), and monitor progress. Your efforts will be crucial in helping clients achieve their recovery goals and improve their quality of life.

Primary Duties and Responsibilities:
Comprehensive Assessment and Service Plan Development: Conduct thorough assessments of clients with serious mental illness to identify their needs, strengths, and challenges. Develop personalized service plans (ISPs) that are tailored to each client's unique situation, incorporating a holistic approach to care. Coordination of Care: Serve as the primary point of contact for clients, coordinating all aspects of their care, including mental health services, medical care, housing, employment, and social support. Ensure that services are integrated and aligned with the client's goals. Monitoring and Documentation: Regularly monitor clients' progress toward their recovery goals. Maintain detailed documentation of all interactions, interventions, and updates to the service plan, ensuring compliance with agency policies and state regulations. Advocacy and Support: Advocate for clients' needs within the community and among service providers. Provide ongoing support and guidance, empowering clients to take an active role in their recovery. Crisis Intervention: Respond to crises as they arise, providing immediate support and intervention to ensure the safety and well-being of clients. Work with emergency services and other resources as necessary to manage crises effectively. Collaboration with Multidisciplinary Teams: Work collaboratively with psychiatrists, therapists, social workers, and other professionals to provide comprehensive care. Participate in regular team meetings to review client progress and adjust service plans as needed. Professional Development: Engage in ongoing training and professional development to stay current with best practices in SMI care and case management. Minimum Qualifications and Competencies: Education: Bachelor's degree in a behavioral health-related field required. One year of experience working in behavioral health, particularly with SMI populations, is preferred. Bachelor's degree in a non-behavioral health-related field with 30 semester hours in behavioral health education and two years of behavioral health work experience. Associate's degree in a behavioral health-related field with four years of behavioral health work experience. Associate's degree in a non-behavioral health-related field with 30 semester hours in behavioral health education and five years of behavioral health work experience. High school diploma or equivalent with 18 credit hours of post-high school education in a behavioral health field completed within the last four years and two years of full-time behavioral health work experience, or four years of behavioral health work experience. Experience: Experience working with seriously mentally ill (SMI) populations is highly preferred. Experience in community-based mental health programs and case management is a plus. Skills and Competencies: SMI Knowledge: Strong understanding of serious mental illness, including symptoms, treatment options, and recovery strategies. Communication: Excellent verbal and written communication skills, with the ability to engage effectively with clients, families, and multidisciplinary teams. Crisis Management: Ability to respond quickly and effectively to crises, ensuring the safety and well-being of clients. Organization and Time Management: Strong organizational skills with the ability to manage multiple cases and tasks simultaneously. Cultural Competence: Ability to provide culturally sensitive and competent care to diverse populations. Professionalism: Commitment to ethical standards, maintaining a professional demeanor, and demonstrating compassion in all interactions. Licenses/Certifications: Valid driver's license with the ability to be insured under an automobile insurance policy. Ability to obtain a fingerprint clearance card, CPR, and First Aid certification.
