Social Worker 2 - Adult Er

Provides psychosocial assessment and intervention to assist patients, families including clinically complex. Communicates, collaborates with the work team using a multidisciplinary approach. Provides linkage to community resources to facilitate optimal care. Provides age specific care to patients, families in all age groups. Develops and implements a comprehensive plan based on identified psychosocial needs and, or discharge needs. Maintains a current working knowledge of UH network services and promotes utilization of such services. Provides guidance, consultation to peers, team regarding routine psychosocial issues and discharge planning. Provides supervision to social work interns if LISW eligible, completion of supervision hours. Personal computer skills. Theoretical and clinical knowledge regarding health care delivery. Multi tasks and prioritizes work. Works autonomously. Communicates effectively with persons of various backgrounds. Monday-Friday with rotating weekends and holidays This position for full time nights in the Adult Emergency Department The shifts can be (3) 12's and (1) 8 hr, or (4) 10's Qualifications: LSW - Social worked - Licensed Master Degree in Social Work. Hospital, Heath Agency (2 years hospital experience preferred).
