A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.
What You Will Do Provides psychosocial assessment / intervention to assist patients/families.
• Provides age specific care to patients/families in all age groups.
Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.
• Develops plans with attention to individual patient and family goals; Provides linkage to community resources to facilitate optimal care
Communicates/collaborates with the work team using a multidisciplinary approach Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Qualifications: Education Bachelor's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience. (Preferred) Knowledge, Skills, & Abilities Computer skills (Required proficiency) Theoretical and clinical knowledge regarding health care delivery (Required proficiency) Multi-tasks and prioritizes work (Required proficiency) Communicates effectively with persons of various backgrounds (Preferred proficiency) Licenses and Certifications Licensed Social Worker (LSW) (Required) Position Details
Previous social work experience is strongly preferred. Prior work in an emergency room or urgent care is also preferred. Schedule is: Monday-Friday with rotating weekends and holidays. This position for full time days in the Adult Emergency Department. The shifts can be (3) 12's OR (4) 10's.
