Seeking a candidate with experience in case management to provide direct case management services to trauma-affected refugees and immigrants with our Refugee Health and Wellness Program. Preference for fluency in a language spoken by the communities we serve. Part-time, 25 hours/week, competitive hourly wage, prorated paid time off and paid holidays, flexible spending account (FSA) program, 403b tax-deferred retirement savings option, direct deposit, training opportunities, availability during office hours of Monday-Friday 8:30-4:30 with occasional evening and weekend availabilityPlease note that this program is scheduled to be funded through 9/29/2028. The position will be reviewed on an annual basis and will be renewed based on continued community need and the availability of funding. Job DescriptionUtilize a trauma-informed strengths-based approach to work collaboratively with the program team, agency staff, and partnering organizations to coordinate services for trauma-affected refugees and immigrantsConduct person-centered, culturally-considered, and trauma-informed wellness assessments with program participantsIdentify client's risk and protective factors - including their strengths, needs, abilities, and preferences - through holistic assessmentRespond to immediate mental health crises, provide stabilization, and connect individuals with emergency services if neededWork with program Social Worker and clients to develop personalized care plans to ensure access to medical, mental health, and social service programsPerform case management services for clients in a variety of topics including but not limited to county benefits, food stability, health care access, employment, childcare, and other identified needs that may be a barrier to mental health treatmentMaintain regular contact with clients, including regular home visits, to monitor progress and address emerging needsIdentify ongoing client needs and coordinate with program Social Worker to ensure receipt of available services and resourcesAdvocate on behalf of clients to ensure access to appropriate resources, benefits, and servicesCoordinate and/or provide transportation to and from program activities, appointments, and other essential servicesCollaborate with agency staff and external care providers to ensure seamless service delivery and care coordinationUphold clients' rights, privacy, and dignity and adhere to all contractual regulationsParticipate in regular staff meetings, serving as an engaged team member, contributing to program development and problem-solvingEngage in ongoing professional development and self-care to maintain personal well-being and high-quality service deliveryMaintain timely and accurate documentation of services delivered, client progress, and any other documentation as required by ORR in a timely mannerMaintain confidentiality of client protected informationCollaborate with student interns in the provision of program services, offering mentorship and guidancePerform other related duties as assigned to help meet the goals of the program and agencyRequirementsMinimum of a High School Diploma or GED, bachelor's degree in social work or a related field preferredSkilled in interpersonal and intercultural communicationAbility to be self-directed with position responsibilitiesStrong organizational skillsAdvanced skill with Microsoft Office and Google Apps including Google Drive, e-mail, and internetPossess professional/academic abilities in English across all skill areas including clear pronunciation in speaking and correct grammar and spelling in writingExperience facilitating partnerships with community medical and social service providersParticipate in a required background checkValid driver's license, reliable vehicle, and current automobile insurance with clean driving record and willingness to drive clientsExperience working with diverse populations and limited English-speaking communitiesExperience working with an interpreter preferred, but not requiredFluency in a language spoken by the communities served is strongly preferred, but not requiredWork Experience and Education Preferences:Prefer bachelor's or other four-year undergraduate degree in a social service, social work or health-related fieldExperience providing case management to individuals and familiesExperience working with interpretersCRIS IS AN EQUAL EMPLOYMENT OPPORTUNITY EMPLOYER.
