Job Title: Occupational Therapist - Home Health Job Specialty: Therapy/Rehabilitation - OT Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: 40 hours per week, Monday-Friday Experience: Minimum of 2 years required License: Valid WI License required Certifications: BLS, NBCOT Must-Have: - Adequate transportation for traveling during the day- Lymphedema experience and certification welcomed- Flu Vaccine required (Medical/Religious Exemptions Only) Job Description: - Provide home health occupational therapy services to patients within a service area of 35-40 miles- Ensure flexible scheduling and demonstrate independent working autonomy- Collaborate with a team including 1 Certified Occupational Therapy Assistant (COTA), 2 Physical Therapists (PTs), and 2 Physical Therapist Assistants (PTAs)- Attend an 8-hour orientation- Dress in business casual attire - scrubs are optional- Adapt to client needs and demonstrate great teamwork skills- Complete required modules during orientation- Willingness to float within hospital as needed
