A Brief Overview
The Transplant Outreach Coordinator works together as an integrated team maintaining positive relationships within the Transplant Institute, University Hospitals Health System, and Community Partners. An outreach/marketing strategy is developed and followed to improve the visibility of the Transplant Institute within the community. This role will support a streamlined process with internal an external partners to increase patient referrals to the Transplant Institute and optimize both patient and referral source experience through the patient care journey. Some local travel is required.
What You Will Do Initiates and maintains positive relationships with patients/customers, referring physicians, and /or clinics, payers, and supporting transplant staff Works together as an integrated team and maintains positive relationships Takes responsibility for self-development and supports a learning environment Displays commitment to the mission of the hospital and its values In collaboration with the Transplant Administrator and other outreach team member(s), design, implement and maintain an outreach plan for the Transplant Institute, with organ specific focus for the purpose of increasing the number of patients who are referred for treatment of their organ failure Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Qualifications: Education Bachelor's Degree In communications, marketing or associated discipline (Required) or Bachelor's Degree Nursing (Required) Work Experience 5+ years Of healthcare marketing and/or communications (preferably transplant and/or organ donation experience) (Required) or 5+ years Nursing experience in transplant and/or organ donation (Required) Knowledge, Skills, & Abilities Demonstrated the development and execution of strategic initiatives with action plans.
