Job Description & Requirements Specialty: Occupational Therapist Discipline: Therapy Start Date: 09/23/2024 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel T4H Job ID: Job Title - OT Case Manager Home Health MUST HAVE:- 1 year OT Home Health experience - active BLS and IL OT license in hand at time of submission - must have Masters degree in OT. Support on team consists of manager/supervisor, lead, and scheduler. EPIC EMR computer chating; will receive Advocate issued laptop. Any scrub color is acceptable as long as it is appropriate. Traveler MUST provide their own transportation to and from all patient visits. Mileage throughout the day may be submitted for reimbursment.
