About Kings Against Violence Initiative (KAVI) KAVI is a nonprofit, 501(c)(3) youth-serving organization with a mission to prevent and eliminate interpersonal violence from the lives of young people through advocacy, peer leadership, community mobilization, and social justice. KAVI promotes community wellness and healing by operating high-impact youth violence prevention and intervention programs in Brooklyn. Our programs take place in hospital-based, school, and community settings. We are a fast-paced, rapidly growing nonprofit organization. About our Victim Services: All KAVI team members are expected to carry themselves professionally and in the best interest of the communities we serve.  KAVI is a community empowerment organization that operates programs grounded in Social Equity, Public Health, Restorative, and Social Work practices. This approach includes our use of Trauma Informed Care, which is used to build trust and promote agency and healing. Our community program is the newest component of our programming. We operate community programs using a two-pronged approach: engagement and services.  We create community engagement programs that bring family and neighbors together to discuss and act on violence prevention. Our programs help community members advocate for our youth, understand and practice restorative justice, and participate in collective healing. Our engagement work provides individuals and communities with the resources they need to heal from traumatic, violent experiences. Healing Circles, advocacy, counseling, linkages to stop-gap services, and assistance navigating human service programs are parts of our Victim Services work. This position will work in tandem with partner agencies, social service providers, and community groups to create a unique support structure tailored to KAVI's participants of care. The Case Manager will supervise Case Workers and Program Assistants in the absence of the Program Coordinator. Additionally, the Case Manager once assigned an individual will serve as the main point of contact for that individual who is interested in receiving support.
DUTIES AND RESPONSIBILITIES: Conduct assessment, and follow up of each client  Complete expanded assessment of clients and family needs during subsequent follow-up evaluation. Maintain documentation of all activities, qualitative data, and services provided for all program participants. Communicate with the Program Coordinator and staff about client follow-up weekly. Provide timely reports on participation, services, and outcomes. Maintains a referral database and system for the referral, coordination, and evaluation of wraparound services Help to determine client's eligibility for assistance by working with client to complete agency application and obtain all required documentation to verify assistance needed. Assist clients in completing intake forms and gathering necessary documentation, and where appropriate, grant clients financial assistance. Recognize and adhere to professional roles and boundaries, able to assess client's capacity to carry out mutually decided upon, realistic goals. Participate in weekly individual supervision meetings and weekly and monthly team meetings. Act as an advocate: investigates and reports adverse occurrences and outcomes and performs intervention as necessary Conduct quarterly case management updates up to one year; access participants' progress through a case plan. Knowledge, Skills, and Abilities: Excellent verbal communication skills. Good writing skills and the ability to maintain correspondence with various agencies and clients  Highly developed ability for problem-solving. Knowledgeable in case management principles, procedures, and practices. Demonstrates adaptability to perform various duties, occasionally changing from one task to another of a different nature without loss of efficiency or composure. Competent computer skills, including the use of Microsoft Office / Google  Ability to maintain confidentiality Ability to maintain integrity and professional rapport towards hospital staff and other KAVI members Excellent time management skills, prioritizing, coordinating, and organizing multiple activities.  Flexibility and willingness to adjust and respond to changing situations.   Interest in social and restorative justice, passion for serving communities
Education and Qualifications: A degree or certificate in a specialized field of work such as Case Worker, Counseling, healthcare, related field. Must have at least one year of experience in delivering human services and/or case management specific to the needs and abilities of the identified program population. Compensation The starting wage for the position is budgeted for an hourly rate of $40.
