A growing healthcare clinic is seeking a Care Coordinator to support patient care by ensuring patients are up to date with their health needs.

Responsibilities Include: Communicating with patients to schedule appointments and follow-ups. Ensuring patients are informed about their treatment plans and medical advice. Handling patient inquiries and resolving issues related to their care. Coordinating with medical staff to ensure seamless patient care. Updating and maintaining patient records accurately. Assisting with patient education and providing relevant health information. Monitoring patient progress and reporting any concerns to the medical team. Supporting the Care Coordinator Manager in executing patient care strategies. Ideal Qualifications: Fluency in Haitian Creole and English. Excellent communication and interpersonal skills. Strong organizational and multitasking abilities. Previous experience in a healthcare support role is preferred. Proficiency in using medical software and office applications. Ability to handle sensitive information with confidentiality and professionalism.

Salary: $25/Hour
