Care Coordinator (Msw)

*Responsibilities Include:*
* Performs resident evaluations as required by state regulations
* Support state regulations for admission and retention of residents
* Assists residents and families with the move in process
* Orients residents and families to the facility
* Assists and interacts with residents and families to access services
* Discharges planning and assistance with the move out process
* Participates in development and implementation of each resident's individual service plan
* Communicates with other members of the management team to ensure coordination of services to residents
* Documents case management notes in residents' files as necessary
* May perform other duties as assigned

*Requirements:*
* Master's degree in Social Work required
* Must obtain license within the first year of being hireed

_Please note- The annual base salary range is $55k to $70k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer._
