HIRING URGENT!!A leading community medical center in San Fernando is looking for a Director of Care Management to join their team!!Description:+ DCM will be overseeing a team of about 15 employees-- 4 MSW/LCSW/LMFTs, 2 managers with teams of case managers and community health workers. + DCM time will be distributed anywhere in the range of 50 to 60 percent of time on administrative duties and the remaining time as a provider. + Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs. + Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.+ Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.+ Plan discharge from care facility to home or other care facility. + Modify treatment plans to comply with changes in clients' status.
