*Responsibilities*:
* Provide comprehensive nursing care to hospice patients in their homes across the New York boroughs
* Conduct thorough assessments of patients' physical, emotional, and psychosocial needs
* Develop and implement individualized care plans in collaboration with an interdisciplinary team
* Administer medications, treatments, and other interventions as prescribed
* Educate patients, families, and caregivers on disease processes, symptom management, and end-of-life care
* Ensure accurate and timely documentation of patient assessments, care plans, interventions, and outcomes
* Coordinate care with physicians, social workers, chaplains, and other healthcare professionals to provide holistic care
*Requirements*:
* Valid Registered Nurse (RN) license in the state of New York
* Minimum of 1 year of clinical experience in hospice, palliative care, or home health nursing
* Excellent communication and interpersonal skills
* Ability to work independently and manage time effectively
* Compassionate and empathetic approach to patient care
* Willingness to travel throughout the New York boroughs to visit patients in their homes
