Position Summary:
The Population Health Coordinator (PHC) is an integral member of the Health Center's Population Health Team and supports the Health Center and its patients to achieve enhanced clinical quality outcomes through quality improvement and intradepartmental coordination. The PHC will accomplish this through updating the electronic health record (EHR) with quality data, performing EHR chart review, creating and utilizing patient registries, tracking patients to ensure test and appointment completion, and outreaching to patients. The PHC plays a key role in the Health Center's Quality Contract and practice improvement initiatives by working closely and routinely collaborating with Population Health team members, Medical Assistants (MAs), Nurses, Providers, operations staff, as well as, external practices, in order to track care and maintain accurate data in the EHR.
Objective
The Quality, Performance Improvement, and Population Health Coordinator supports key initiatives aimed at improving patient care, enhancing clinical outcomes, and promoting population health. This role offers hands-on experience in the analysis of healthcare quality metrics, development of performance improvement initiatives and collaboration with interdisciplinary teams to drive organizational goals related to healthcare excellence and overall organizational quality improvement.
Primary Responsibilities:
1. Data Collection and Analysis:
a. Assist in gathering and analyzing healthcare quality data, such as clinical outcomes, patient satisfaction, and operational performance metrics
b. Work with teams to identify trends and areas for improvement based on data insights
2. Quality Improvement Initiatives:
a. Participate in performance improvement initiatives focused on enhancing healthcare processes, reducing waste, and improving care delivery
b. Assist in monitoring progress and reporting on key performance indicators (KPIs) for ongoing improvement initiatives
3. Population Health Initiatives:
a. Support initiatives aimed at improving the health outcomes of specific populations, with a focus on prevention, wellness, and addressing social determinants of health
b. Work with interdisciplinary team members to coordinate programs that aim to reduce health disparities and obtain quality metric goals aligning with the organization's ACO
c. Conduct targeted outreach to patients for per health topics and utilize the electronic health record system reporting functionalities to monitor, track and aim to complete care gaps within the patient population
4. Interdisciplinary Collaboration:
a. Collaborate with clinical, administrative, and operational staff to ensure that performance improvement strategies are aligned with patient care goals
b. Participate in team meetings and contribute to discussions on quality improvement strategies and population health management
5. Documentation and Reporting:
a. Assist in the preparation of reports, presentations, and documentation for performance improvement and population health initiatives
b. Ensure compliance with regulatory requirements and quality standards
6. Other duties as assigned.
Qualifications/Skills:
1. Currently pursuing a degree in public health, healthcare administration, nursing, or a related field
2. Strong analytical skills and ability to interpret healthcare data
3. Excellent written and verbal communication skills
4. Detail-oriented and organized
5. Ability to work independently and collaboratively within a team environment.
6. Familiarity with
a. Healthcare quality metrics, such as HEDIS, CMS, accountable care organizations, or other performance measurement tools, is a plus
Bonus Skills & Qualifications
1. Bilingual - Haitian Creole and/or Spanish preferred
2. Previous healthcare experience
3. Applicant seeking an internship for 6+ months
