Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.
Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. Compensation Range: $16/hr - $17/hr commensurate with experience.
Overview:
Prevention Navigator for SAMHSA HIV Prevention Grant.
Responsibilities:
Assist the program director and lead navigator with all administrative tasks related to the project.
Data collection for the program (SAMHSA Youth Questionaries, EBP's Assessments, pre & post-tests, evaluations, others) data entry and data analysis.
Conduct mapping, outreach, recruitment, and linkage to care activities (HIV/HCV/SUD treatment as needed).
Conduct pre and post HIV test counseling and other risk assessments.
Assist in accompaniment, transportation, follow up services and support.
Assist and development of collaborative agreement with other community-based organizations and partners.
Contact schools, educational programs or different spaces where small groups can be identified to offer Cuidate interventions.
Preparing HIV prevention and harm reduction kits
Monitor available inventory and notify lead when there is a need for project materials.
Provide internal/external referrals and follow up
Assist and provide ongoing support to encourage retention in services
Represent project and agency in different forums.
Update the calendar or work plan
Reproduce educational materials, questionnaires, pre- and post-test sheets, evaluations, attendance sheets, etc.
Coordinate and conduct educational training to different organizations or population groups.
Ensure quality, accurate and detailed documentation of all program services.
Support the monitoring and compliance with project goals and objectives.
Stock the cruising spot and other prophylactic dispensers with prevention materials.
Update the profile (gender, age group, other) of the groups or individuals impacted by educational activities or materials.
Assist in the creation and maintenance of folders, lists or records pertaining to the project.
Job Requirements
BA in human behavior and work experience within the social service field will be taken into consideration)
Experience with gathering and reporting data
Experience with Microsoft 365
Meticulous at maintaining records and confidentiality protocols
Highly motivated to work with people who experience homelessness, substance use, and/or co-occurring disorders in an outreach setting
Able to work closely with others (team player)
Good written and verbal communications skills
Experience with youth at risk a plus
Nontraditional outreach experience a plus
Patient Navigation Certification a plus
Own car and valid license
