RN Outbreak Management - $48 per hour Location area (SPA 7): Downey, Norwalk, Bellflower, Cerritos, Pico Rivera, Whittier Location area (SPA 8): Torrance, Carson, Inglewood, Long Beach, Lomita, Lawndale Amergis Healthcare Staffing is seeking a Disease Management Care Coordinator (DMCC) to assess member's current health status and understanding of their chronic condition. The DMCC provides education to assist with self-management, provides information on network access, and identifies and implements interventions and resources that address and close gaps in care.Essential Duties and Responsibilities: Promotes member program enrollment Conducts follow up assessments and evaluations of members Responsible for proper documentation of assessments/evaluations Promotes a patient/provider relationship Prioritizes care needs and develops plans of care to address gap and/or barriers Assists in coordination of treatment plans Collaborates with member regarding opportunities for optimizing care Evaluates and monitors member's response to treatment plans Assists in facilitating member understanding of physician's treatment plan Modifies treatment plans accordingly with medical doctors Coordinates the acquisition and proper use of medical equipment Identifies gaps or barriers that would prevent the member from following through with a recommended treatment plan consistent with optimal care for specified disease condition Identifies and implements interventions and resources that address and close gaps in care Performs other duties as assigned/necessary Minimum Requirements: Current /active professional nursing license required Three (3) years of clinical experience within managed care environment preferred Ability to quickly identify and prioritize member needs Strong level of critical thinking and analytical skills Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be required Computer proficiency required Must be at least 18 years of age Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: Competitive pay & weekly paychecks Health, dental, vision, and life insurance 401(k) savings plan Awards and recognition programs *Benefit eligibility is dependent on employment status.
