Rn Care Manager - At Home

About the Role We are seeking a skilled RN Care Manager to join our team at Providence Medical Group. As a key member of our care coordination team, you will play a vital role in providing high-quality, patient-centered care to our most medically fragile patients.
Key Responsibilities Provide comprehensive care coordination services to patients and their families, including assessment, planning, implementation, and evaluation of care. Collaborate with healthcare providers, community resources, and patients to develop and implement individualized care plans. Conduct home visits to assess patient needs and provide education on self-management and disease prevention. Work closely with our team of physicians and other healthcare professionals to ensure seamless transitions of care. Document patient information and outcomes in a timely and accurate manner. Requirements Oregon Registered Nurse License upon hire. 2 years of clinical medical-surgical, acute care, or long-term care experience, including 1 year in community-based/ambulatory care and 1 year in care of the frail elderly. Preferred Qualifications Bachelor's Degree in Nursing. Home health, hospice, palliative care, managed care, or skilled nursing facility background. 1 year of care management experience with diverse patient populations.
