We are looking for a Social Services Manager that be crucial in supporting patients by connecting them to local services such as housing, behavioral health resources, transportation, and more. Additionally, the Social Services Manager will assist patients in enrolling in Medicaid, ensuring they have access to essential healthcare services.
What you'll do in the role: Patient Assessment: Conduct comprehensive assessments of patients' social, emotional, and environmental needs based on experience from the existing care team. Coordinate and conduct thorough follow-up assessments and interventions for patients experiencing crises, ensuring comprehensive support and continuity of care. Service Coordination: Identify and connect patients with appropriate local services, including housing, food options, behavioral health resources, transportation, etc. Medicaid Enrollment: Assist patients in understanding and completing the Medicaid enrollment process, ensuring all necessary documentation is submitted and managed to completion. Resource Management: Maintain an up-to-date directory of community resources and services available to patients. Collaboration: Work closely with healthcare providers, case managers, and other social service organizations to coordinate care and ensure a "wow" approach to patient support. Advocacy: Advocate on behalf of patients to ensure they receive the services and support they need. Documentation: Accurately document all patient interactions, assessments, and service referrals in the AndHealth app, and potentially the EHR. Follow-Up: Provide ongoing follow-up with patients to ensure they are successfully connected to services and support their continuous engagement with needed resources and contribute towards patient satisfaction metrics by solving for the patient's largest needs. Education & Licensure Requirements: Bachelor's or Master's degree from an accredited institution. A CCM is preferred. An LSW license in the state of Ohio is preferred. Other Skills or Qualifications: Minimum of 2 years of experience as a social worker, preferably in a healthcare or community services setting in and around Columbus. Familiarity with local community resources and services and understanding of Medicaid enrollment processes. Excellent interpersonal, communication, and organizational skills. Ability to work independently and as part of a multidisciplinary team. Proficiency in using electronic health record (EHR) systems and adapting to new technologies as needed. And Bonus points if you have: Experience working with diverse populations and understanding of cultural competence in service delivery. Bilingual or multilingual skills.
