Job Description:CVHCare is looking for a qualified Physical Therapist. Assigned county Territory is Area #4 (Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafeyette, Orinda, Pleasant Hill, Walnut Creek) and Area #5 (Pittsburg, Discovery Bay, Antioch, Brentwood, Oakley, Bethal Island, Bay Point)Additional travel outside area may be required. Weekend coverage may be required.If Guarantee, the following shall apply: Productivity: At least 27 points per week (points described below). 36 hours Guarantee. Traveler must complete 30 points in the week. Must be willing to travel outside the area or work weekends if required to maintain weekly visit expectations. If Traveler refuses patients, guarantee does not apply. Guarantee does not apply for 1st and last week of assignment, or for weeks in which there are holidays, or for weeks in which the traveler has time off.If Non-Guarantee, the following shall apply: To secure a full caseload, Traveler may be required to go outside the service area occasionally. Can refuse patients. No guaranteed hours. Traveler must be willing to see Covid+ and Covid Pui patients. Payment made on per visit basis. Any lost, stolen or damaged equipment will be the sole responsibility of the Agency to fully reimburse the Facility. Facility data plan is optimal for full time field clinicians (documentation, texting, calls, hotspot). Data usage will be kept to a strict minimum, reasonable and necessary for business. Facility has established a monthly max limit of 3 GB per user.
